Agreed, the biggest blunder for SEO is backlinking to the wrong sites, i.e., Google can read you site as four different sites, and when you backlink to a your site you what to make sure to link to the same one every time. Although the sites are the same Google will read them as different site, here, xyz.com is different then xyz.com/ although it is the same site.

Dup Content is being confused with the cookie cutter approach that Google is talking about in their guidelines. Adding pages with the same content to your site to increase the size of the site to influence rankings is the cookie cutter approach and that is what Google is talking about when it come to dup content.

I think that bad title is also a big mistake, because the title on your page has to have a close connection to the content. The ranking will be assigned only based on the best matches.
